Bringing Augmented Reality Guidance Directly into the Operating Room

Case Study | Orthopedic Surgery, AR & Surgical Precision

    Overview

In orthopedic surgery, precision is everything. Surgeons must visualize complex anatomical relationships while maintaining constant focus on the surgical field. Traditionally, this has required looking away at monitors, relying on assistants, or using bulky navigation systems—each adding friction, cost, or cognitive load.

NextAR, a surgical augmented reality platform developed by Medacta, partnered with Vuzix to bring real-time, patient-specific surgical guidance directly into the surgeon’s field of view using Vuzix Blade™ smart glasses. The goal: improve surgical accuracy, reduce distractions, and enhance outcomes without disrupting natural surgical workflow.

    The Challenge

    Orthopedic procedures—particularly joint replacements—require:

    • Accurate alignment and placement

    • Continuous visualization of biomechanical data

    • Minimal distraction during critical steps

    • Hands-free access to guidance

    Traditional navigation systems often require:

    • Large capital equipment

    • Additional OR staff or operators

    • Surgeons to look away from the patient

    • Increased setup time and cost

Surgeons needed a way to access guidance without diverting their eyes or hands from the procedure.

The Solution: NextAR + Vuzix Smart Glasses

NextAR created a see-through augmented reality surgical platform that overlays patient-specific guidance directly into the surgeon’s natural line of sight using Vuzix Blade smart glasses.

    How It Works

    1. Pre-operative planning

      • CT scans are processed through proprietary algorithms

      • A personalized biomechanical model is created for each patient

    2. Intra-operative AR guidance

      • Surgeons wear Vuzix Blade smart glasses

      • Real-time guidance appears directly over the patient

      • Alignment, angles, depth, and positioning data are displayed live

      • Surgeons never need to look away from the surgical field

The result is a seamless fusion of digital intelligence and real-world anatomy.

    Designed for the Operating Room

Vuzix Blade smart glasses were selected specifically for surgical environments because they are:

    • Lightweight and comfortable for long procedures

    • Hands-free, with voice commands for interaction

    • See-through, preserving full situational awareness

    • Durable and ergonomic, designed for all-day wear

As Dr. Wes Cox, Orthopedic Shoulder & Elbow Surgeon and Section Chief at UAMS Orthopaedics, explains:

“Having Vuzix glasses gives you the truth. Immediately you know that not only is this what you intended to do, but this is what you’re about to do.”

    Zero Distraction, Maximum Focus

One of the most critical advantages of the NextAR platform is cognitive simplicity.

    Surgeons do not need to:

    • Look at external monitors

    • Shift attention away from the patient

    • Rely on verbal cues from others

Instead, all relevant information appears exactly where and when it’s needed, allowing surgeons to maintain continuous focus on the operative field.

As Dr. Cox noted, after a brief learning curve, using the glasses feels no different than normal surgery—except with “extra real-time feedback.”

    Clinical Benefits

The NextAR + Vuzix solution delivers meaningful advantages:

    Improved Surgical Accuracy

    • Patient-specific biomechanical models

    • Real-time tracking and alignment feedback

    • Reduced variability between planned and actual outcomes

    Reduced OR Complexity

    • No bulky navigation towers

    • No additional equipment operators

    • Lower setup time and footprint

    Enhanced Training & Education

    • Surgeons can see exactly what they’re doing, with immediate feedback

    • Ideal for teaching complex procedures

    • Visual learning accelerates skill acquisition

    Expanding Across Specialties

While initially focused on orthopedic joint replacement, the NextAR platform is now being used across multiple surgical specialties, including:

    • Shoulder replacement

    • Knee replacement

    • Spine procedures

The same AR-driven approach adapts to different anatomical models and surgical workflows.

The Future of AR in Surgery

According to Dr. Cox, augmented reality has enormous potential beyond guidance alone:

    • Improved surgical education

    • Real-time collaboration

    • Better communication with anesthesiology and OR staff

    • Lower hospital costs by reducing reliance on large navigation systems

He emphasizes that AR doesn’t replace surgical skill—it enhances it, allowing surgeons to make better decisions with greater confidence.

    Conclusion

The collaboration between NextAR and Vuzix demonstrates how augmented reality can be seamlessly integrated into real surgical workflows. By delivering real-time, patient-specific guidance directly into the surgeon’s field of view, the solution improves precision, reduces distraction, and elevates both surgical performance and education.

This case study shows that AR-guided surgery is no longer theoretical—it’s already improving outcomes in operating rooms today.
